A Social Day Program is a program of care during
the day, in a supervised, structured setting away
from the adult's home.

Social Day Program is designed to meet the needs
of adults with functional impairments  through a
plan of care in a protective setting.
The program provides socialization as well as
activities to maintain and increase
mental and physical abilities.

Specializing in memory loss and related illnesses,
we accept Alzheimer's clients that do not require
advanced skilled nursing needs.

HOME HEALTH CARE / NURSING

Home Health Care is the provision of personal
care services, such as help with bathing,
dressing,assistance with ambulation, meal prep,
shopping, light housekeeping and
companionship.

These services are provided by certified Home
Health Aides and Personal Care Aides, under the
supervision of a Registered Nurse.

Our services are provided in the home, in
hospitals, and in other institutions. We meet the
needs of  the  handicapped, disabled and
convalescent adult clients. Services are available
from two hours to twenty-four hours a day, seven
days per week. We abide by all New York State
Rules and Regulations.

An admission assessment includes an evaluation
of the potential client and the home environment
by a Registered Nurse to identify home care
needs.

The provision of care in the home will be for the
purpose of restoring or maintaining maximum
levels of comfort, function and health for each
individual.
CASE MANAGEMENT

Case Management is the oversight and
coordination of care for seniors who live
independently or with family.

The Case Manager can schedule and coordinate
services such as medical and
diagnostic appointments and arrange
transportation.
The Case Manager evaluates
the needs during the initial visit and makes
referrals to various health care providers and
community organizations,i.e. Orange County
Office for the Aging, Jewish Family services and
Alzheimer's Assocation.

Respite / Housekeeping in home services include
companionship, shopping, cooking, light
housekeeping and laundry.

These services may be of benefit for the primary
caregiver who wishes some relief from the many
responsibilities of caring for an ill or aging  loved
one.

Seniors who are unable to do these things for
themselves may also benefit from this service.

A full service residential cleaning plan designed
to fit your needs. Offering daily, weekly,
bi-weekly and monthly service as well as
one time cleanings.*(A minimum of  two
hours per cleaning appointment is required).
